Instructions for Application The deadline to apply for the RQ Assessment will be 31st of May, 2012. Please fill in all of the paragraphs of the Application Form and enclose a recent I.D. photo. Please complete the Curriculum Vitae. Please complete the Logbook (photocopying/scanning if necessary) according to the list of operation categories (Open Operation Category). The Logbook must be signed by the Head of the Department or by the Medical Director of the Hospital Section/ Division. In case of questions, contact your national EBOMFS representative, the Secretary General or the President of the Scientific Committee of EBOMFS. Please prepare 1 copy of your Medical Diploma, your Dental Diploma (if available) and Diploma of Recognized UEMS-Specialty of Maxillofacial Surgery. The copy should be sent either to the Secretary General in Helsinki, or to the President of the Scientific Committee of EBOMFS in Leuven. The fee for the assessment will be EUR 450 €. It must include the required amount of EUR 450 € plus all bank transfer charges. The payment must be remitted only by bank transfer (no cheques, no cash, no credit cards) to the account of European Board of Oro-Maxillo-Facial Surgery. The candidate's name in block letters and email address must be included. Should the candidate fail the C.V./Logbook Assessment, she/he will have a chance to re-apply for the next RQ Assessment without any further costs. If she/he does not re-apply, EUR 300 of the fee will be reimbursed. For this reason, the IBAN code ( see IBAN ) and BIC code ( see BIC ) must be disclosed. If the candidate fails the MCQ and Oral examination, she/he is allowed to re-apply once for the next RQ Assessment, for a payment of EUR 300.Secretary General:
